Create the layout of your news letter form, go to your toolbar > Insert > Form > Text Field

You will have a dreamweaver popup toadd a form tag which is the red dashed line around your form in dreamweaver, click Yes.

Now insert a button, go Insert > Form > Button

You should now have this:
